St. Joseph's Hospital continues to expand its role in the treatment of complex medical and chronic disease, illness prevention, research and education. Our ambulatory medicine teams specialize in the treatment of complex medical and chronic disease conditions with the goal of assisting individuals to reach optimal health and well-being. Our different teams work together to provide patients with comprehensive assessment, diagnosis, disease prevention and management strategies and follow up care.
 
The Cardiac Rehabilitation and Secondary Prevention Program is a six-month program providing care for patients with known heart disease (recent heart attack, angina, angioplasty or heart surgery). The goals of the program are to assist patients to recover and adjust to heart disease, and to provide secondary prevention, emphasizing principles of self-management, through reducing the physical complications of heart disease, improving patients psychological well-being and successfully returning patients back to an active work or social schedule.
 
The Pulmonary Rehabilitation Program is a newly developing program providing care for patients with known chronic obstructive lung disease. The goals of the program are to assist patients to maintain and improve functional abilities through exercise and group programing. There is a strong emphasis on self-management principles in supporting patients.
 
Both programs are currently seeking a Psychologist to provide comprehensive patient care through: assessment of patients psychological and behavioural needs, with diagnosis as appropriate; provision of psychological interventions to reduce psychological or behavioural risk factors such as stress, tobacco use or non-adherence to therapies; providing treatment for psychological conditions, typically including but not limited to adjustment, mood and anxiety disorders; contributing to program development, evaluation and quality improvement; participating in program-related clinical research. The Psychologist will be responsible and accountable for developing and coordinating an individualized psychological care plan with evidence-based interventions in collaboration with the Cardiologist/Respirologist, the care team, patient and family.
