The FOCUS Community Mental Health Outreach Team is an innovative partnership program between St. Michael’s Hospital and COTA Health. The program relies on an interdisciplinary team of mental health professionals to provide a continuum of integrated intensive case management (ICM) and Assertive Community Treatment (ACT) services to individuals with serious mental illnesses to address both immediate and future service intensity needs. Services are organized to allow for flexibility of movement between levels of services (between ICM and ACT) while maintaining continuity of care. As part of the interdisciplinary FOCUS Team, the Psychologist will provide clinical assessment, individual and group evidence-based treatments and evaluation support for the target population.

Position responsibilities include providing consultation to clarify diagnosis and contribute to the clinical management of clients, provision of individual and group therapies, and participation in education/teaching, research, quality improvement and program evaluation initiatives.
